To register for the Hop to the Beat Lindy Hop Competition 2002:
- Complete and submit the following form. Submit a separate form
for each person.
- Once you have submitted the form you will be taken to the Hop
to the Beat Lindy Hop Competition 2002 Waiver page. Each person
will be asked to complete and sign a separate copy at check-in
on the day of the competition. If under 18 years waiver must
be signed by parent or guardian.
- Full payment is required for your registration.
- Mail payment to:

IMPORTANT: The deadline for registering
is May 15, 2002.
You will be registered, and a place held, once payment is received.
Include your email address (or phone number if you do not have email)
on your payment. If the payment is for more than one person, list all names. You do not need to include a copy of the registration form if you submitted it on-line. Competition division sizes will be limited, please register
early to ensure a spot.
On-line banking can also be used to make payment. Please allow
at least 5 days for on-line banking payment to be received.
